Fishbone Diagram
A one-page root-cause worksheet drawn as an Ishikawa fishbone. Name the effect you're diagnosing — it becomes the head of the fish — then sort its drivers into six bones branching off the spine: People, Process, Tools, Environment, Information, and Timing. List a few causes under each, then converge on the one most likely root cause and the fix it points to. The symptom is rarely the cause, and the loudest cause is rarely the root — six fixed bones prompt every angle so nothing gets overlooked. Leave any line blank to fill in by hand.
