Fishbone Diagram

A one-page root-cause worksheet drawn as an Ishikawa fishbone. Name the effect you're diagnosing — it becomes the head of the fish — then sort its drivers into six bones branching off the spine: People, Process, Tools, Environment, Information, and Timing.
